Membership Agreement:

The membership holder agrees to be the exclusive user of this card. This card does not expire, but is subject to change, members may be notified via email of changes. 19+. Valid only at New Amsterdam Cafe, 301 W Hastings St, Vancouver, BC, Canada. We reserve the right to change these benefits at any time.

Please enter all 9 digits of the membership card. YOU MUST BE ISSUED A MEMBERSHIP CARD FROM A STAFF BEFORE ENTERING THIS FORM. All digits must be entered in order for your card to work on the next visit.
